    The medicine is a combination formulation containing Fluticasone and Salmeterol as the active ingredients. Fluticasone, a corticosteroid, is an anti-inflammatory agent of the drug that helps reduce symptoms and exacerbations of asthma and COPD, while Salmeterol, a long-acting beta2-adrenergic agonist, treats constriction of the airways, opening them up for a better airflow. Used concomitantly, these two components facilitate chest tightness, coughing, wheezing and shortness of breath for an easier breathing.

    Advair Octacaps should not be used in acute episodes of asthma or COPD where intensive measures, such as a short-acting medicine, are required.

    Indications

    Systematic maintenance treatment of asthma in patients favorable to the use of a combination drug (long-acting beta2-agonist and inhaled corticosteroid).

    Symptomatic treatment of COPD, including chronic bronchitis and emphysema.

    MOA (mechanism of action)

    Fluticasone, a synthetic trifluorinated corticosteroid which has the ability to decrease the formation and release of endogenous inflammatory mediators such as leukotrienes, histamines, eicosanoids, and cytokines. Its early anti-inflammatory effects also include the inhibition of macrophage and leukocyte movement and activity in the inflamed area by reversing vascular dilation and permeability.

    Salmeterol is a long-acting beta2-adrenergic agonist (LABA) that works by stimulating beta-2 receptors, located on the bronchial smooth muscle in the lungs. It also mediates the activity of adenyl cyclase, an intracellular enzyme responsible for the formation of cyclic adenosine monophosphate (AMP). These actions cause the relaxation of the bronchial smooth muscle, producing bronchodilation and increased bronchial airflow into the lungs. The effect of Salmeterol can last up to 12 hours after a single dose intake, which results in a decreased number of asthma exacerbations and improvement in respiratory symptoms.

    Dosage

    Advair Octacaps come in the form of inhalation powder and are to be administered twice daily (morning and evening, approximately 12 hours apart) by the orally inhaled route only through a breathing device called Octahaler. The capsules must not be swallowed.

    Asthma treatment in patients over 4 years of age:

    • 1 inhalation twice a day;
    • maximum dose is 200 mcg a day.

    Asthma treatment in patients over 12 years of age:

    • 1 inhalation twice a day;
    • maximum dose is 1000 mcg a day.

    COPD treatment in patients over 18 years of age:

    • 1 inhalation twice a day;
    • maximum dose is 1000 mcg a day.

    The medication should be used on a long-term basis for a better outcome, even in the absence of symptoms. Patients should be periodically assessed by their doctor to evaluate the continuing benefits of the treatment.

    How to take Advair Octacaps using the Octahaler device:

    • Remove the protective cap and twist the mouthpiece following the direction of arrows on your Octahaler.
    • Take one octacap and place it in the capsule chamber.
    • Close the capsule chamber by turning the mouthpiece back to its closed position.
    • Hold the Octahaler between your thumb and index finger on the red buttons.
    • Press both buttons together with a quick and firm pressure to puncture a capsule inside the capsule chamber.
    • Breath out deeply and insert the Octahaler’s mouthpiece into your mouth gripping it firmly with your lips.
    • Tilt your head slightly back and inhale steadily and as deeply as you can.
    • The capsule inside will make a rattling sound that serves as an indication that the Octahaler delivers the medicine into your lungs.
    • Remove the device from your mouth and hold the breath for at least 10 seconds or for as long as is comfortable.
    • Breath out gently through your nose.
    • Open the mouthpiece to discard the empty capsule.
    • If you find out the capsule is not completely empty and some medicine remaining in the capsule, close the mouthpiece and repeat the steps from 6 through 10.
    • After a needed dose intake, rinse your mouth with water to avoid any infections.
    • Remove the used capsule from the capsule chamber and put the protecting cap back on the Octahaler.

    How to clean your Octahaler device:

    • Open the Octahaler by turning the mouthpiece as far as possible in the direction of the arrows.
    • Rinse the mouthpiece with lukewarm water and let it dry completely.
    • Clean the capsule chamber with a little brush. Push the buttons a few times, so the perforation needles can be cleaned as well. Do not use water for the base of the Octahaler and/or the perforation needles.
    • Clean your Octahaler once every week.
    Missed dose

    If you miss a dose of Advair Octacaps you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor’s recommended dose.

    Overdose

    If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at room temperature between 15°C and 30°C (59°F and 86°F) and away from excess moisture and heat. The medication should not be refrigerated. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Use the medicine with caution in patients with pulmonary tuberculosis.

    Do not combine Advair Octacaps with the use of beta-blockers for high blood pressure or other heart conditions treatment (like atenolol, propranolol and sotalol), medicines to treat infections (such as ritonavir, erythromycin, ketoconazole, and itraconazole), diuretics used to treat high blood pressure (like furosemide), other bronchodilators (such as salbutamol), xanthine medicines (e.g., aminophylline and theophylline), HIV drugs (ritonavir, cobicistat).

    Do not use the drug if you have severe liver problems.

    Do not take the medication if you are pregnant or breastfeeding without consulting your doctor.

    Do not give the medicine to children under 4 years of age without your child's doctor advice.
