    Minirin® Melt is a medication used to treat primary nocturnal enuresis (bedwetting) in patients from 6 to 65 years of age whose ability to concentrate urine is normal. Minirin® Melt is administered in the treatment of cranial diabetes insipidus, a type of hypothalamic or pituitary disorder characterized by the production of large amounts of urine and constant thirst.

    Description

    Minirin® Melt is manufactured by Ferring International Center SA and marketed by Ferring Pharmaceuticals Pvt. Ltd. It belongs to the class of medications known as antidiuretics. Minirin® Melt, a sublingual wafer, known as an ‘orally disintegrating tablet’, contains the active substance Desmopressin. The medicine helps control excessive urination during the night (bedwetting) and is used to treat central diabetes insipidus, a condition where the kidneys are unable to conserve water due to a lack of antidiuretic hormone (ADH). This medication acts by reducing the amount of urine kidneys can produce, thus maintaining the water balance in the body.

    Indications:

    - cranial diabetes insipidus associated with the excessive urine production and constant thirst;

    - nocturnal enuresis (bedwetting) in patients over 6 years of age who have a normal ability to concentrate urine but do not respond to enuresis alarm methodology of treatment;

    - polydipsia and polyuria following the surgical removal of the pituitary gland (hypophysis).

    The medication is also used to treat hemophilia A and von Willebrand disease, however, is only administered in mild to moderate cases.

    MOA (mechanism of action):

    The active substance Desmopressin is a synthetic version of natural pituitary Vasopressin, an antidiuretic hormone that reduces urine production. Desmopressin has a similar structure to Vasopressin, however, it has a more prominent effect on diuresis, acts specifically on the V2 receptors in the kidneys and has a considerably longer duration of action. Desmopressin works by boosting the permeability of the epithelial layer of the distal tubules and enhancing the reabsorption of water from the primary urine. With neurogenic diabetes insipidus, Minirin® Melt helps increase urine osmolarity and lower its volume (this also decreases the osmolarity of the blood plasma). As a result, patients have a reduction in the frequency of urination and the amount of urine production. It usually starts to work within an hour and lasts for 7 to 9 hours.

    Dosage

    Minirin® Melt is for sublingual use without the need for water. Try not to crush a ‘wafer’ tablet when removing it from the blister. Gently press on the foil to take it out. Do not push it through the blister foil. Place Minirin® Melt ‘wafer’ tablet under the tongue and wait until it dissolves.

    For the treatment of bedwetting (nocturnal enuresis)

    Adults:

    - 120 micrograms under the tongue at bedtime as a starting dose;

    - 240 micrograms under the tongue at bedtime, if the starting dose does not adequately help resolve the bedwetting.

    Children:

    - 200 micrograms under the tongue at bedtime;

    - the dosage may be increased up to 600 micrograms at bedtime if prescribed by a doctor.

    Avoid drinking fluids from one hour before taking Minirin® Melt until the next morning (at least eight hours). Over this period, reduce your fluid intake to just a few sips in avoidance of undesirable side effects. Remember to drink normally throughout the day to prevent dehydration.

    For the treatment of cranial diabetes insipidus

    Adults: 120 micrograms three times a day.

    Children: 60 micrograms three times daily, administered sublingually.

    Consult your doctor about the most suitable dose for you. Remember to keep your body water in balance. Carefully follow your doctor's instruction about fluid intake.

    Missing a dose

    If you miss a dose of Minirin® Melt you should take it as soon as possible. If it is the time for the next dose, skip the missed dose and go back to your regular dosing schedule. Do not exceed the doctor’s recommended dose.

    Overdose

    If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately. Do not exceed the doctor’s recommended dose.

    Storage

    Store at a room temperature below 25°C (77°F) away from excess moisture and heat. Keep Minirin® Melt in the original container and throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Contraindications

    Do not take Minirin® Melt if you are allergic to Desmopressin or any excipients contained in the medicine.

    Do not take Minirin® Melt if you are in the habit of drinking large amounts of fluid.

    Do not use the medicine if you have cardiac insufficiency (shortness of breath, swelling of the feet or legs due to fluid build-up).

    Do not take the ‘wafer’ tablets if you have kidney disease where you pass little or no urine.

    Do not use Minirin® Melt if you have low levels of sodium in your bloodstream.

    Do not take the medication if you have a syndrome of inappropriate antidiuretic hormone secretion (SIADH), released in excessive amounts from the posterior pituitary gland or an abnormal non-pituitary source.

    Do not use the medication if you are taking medicine to stop diarrhoea (loperamide), medications which tend to release antidiuretic hormone (e.g. selective serotonin reuptake inhibitors, tricyclic antidepressants, and chlorpromazine or carbamazepine) or drugs used to treat sugar diabetes, as they can increase the risk of fluid build up in the body.

    Do not take Minirin® Melt in conjunction with non-steroidal anti-inflammatory drugs (NSAIDs), as they may promote water retention and hyponatremia.

    Do not take this medicine while you are breastfeeding or pregnant without consulting your doctor.

    Do not give Minirin® Melt to a child under the age of 6 years and elderly people over 65 years of age.
