    Description

    Cimduo is a combination medication used to treat a long-term infection caused by either HIV-1 or HBV or both HIV-1/HBV co-infected patients. It is used with other antiretroviral medicines to help control HIV-1 and HBV infections and prevent the virus from becoming resistant.

    There are two active ingredients present in Cimduo - Lamivudine and Tenofovir - which act together to decrease the amount of the virus in the blood and increase the number of infection-fighting cells in the body. Lamivudine is a nucleoside reverse transcriptase inhibitor which hinders the process of the viruses formation, while Tenofovir, a nucleotide reverse transcriptase inhibitor, works by reducing their (viruses’) ability to replicate and infect other cells of the body. The administration of this concomitant drug lowers the chance of getting HIV or HBV complications like new infections or even cancer development and boosts the immune system for a better response against any infectious disease.

    It should be noted that this medicine is not a cure for either HIV or HBV infection. The concomitant therapy with Cimduo does not prevent from passing the virus on to other people, therefore, it is important to take precautions in order to avoid infecting the others.

    MOA (mechanism of action)

    The two active substances present in the formulation, Lamivudine, and Tenofovir, are both antiretroviral agents which act by suppressing the activity of reverse transcriptase, a viral DNA polymerase essential for the replication process of HIV and HBV.

    One of the components is Lamivudine, a cytosine analog and an inhibitor of both hepatitis B virus and human immunodeficiency virus reverse transcriptases (RTs). To be effective, Lamivudine undergoes intracellular phosphorylation to its active metabolite - Lamivudine triphosphate (L-TP). This nucleoside analog is later incorporated into DNA that results in the termination of viral DNA chain.

    Another active substance Tenofovir, a nucleotide reverse transcriptase inhibitor, blocks the activity of HBV and HIV-1 reverse transcriptase by preventing the linking of natural substrates to the viral DNA chain, thus precluding its growth. Once Tenofovir is incorporated into a DNA strand, it causes early termination of DNA transcription, disrupting the virus replication and spread.

    Dosage

    Cimduo is a fixed-dose combination product the accurate dosing of which is to be adjusted by a doctor according to the results of patients blood tests. The medication should be administered orally once daily with or without food. Swallow the tablets whole with a glass of water.

    Maximum daily dose of Cimduo - 300/300 mg.

    The concomitant treatment with Cimduo should not be stopped abruptly without a doctor’s notice as it may worsen the hepatitis B virus infection. In case the therapy is discontinued, a close follow-up for a few months, including regular blood testings, is needed in people who have HBV.

    Missed dose

    If you miss a dose of Cimduo you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

    Overdose

    If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at room temperature under 30°C (86°F) away from excess moisture and heat. Do not use Cimduo if the seal is missing or broken. Keep the bottle tightly closed. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Contraindications

    Cimduo is contraindicated in patients with a history of hypersensitivity to Lamivudine or Tenofovir contained in the preparation.

    Do not use any other drugs with one of the active ingredients contained in Cimduo (Lamivudine or Tenofovir).

    Do not take the medicine if you have serious have kidney problems, including end-stage renal disease requiring hemodialysis.

    Use of the medication is not recommended in patients with creatinine clearance less than 50 ml/min.

    Do not take Cimduo if you have or have had inflammation of the pancreas (pancreatitis).

    Do not use the medication if you have bone problems, including a history of bone fractures.

    Be cautious while driving or operating machinery as Lamivudine/Tenofovir tablets may cause dizziness.

    Do not take this medicine during pregnancy or if breastfeeding unless specifically discussed its administrations with your doctor.

    The medication is not for use in children under 12 years of age and the weight not less than 35 kg without a doctor’s recommendation.

    Other medicines

    Do not take Cimduo concomitantly with any of the following:

    • HIV protease inhibitors (like atazanavir, darunavir, lopinavir, and ritonavir)
    • drug combinations that may increase concentrations of Tenofovir (such as lopinavir/ritonavir, atazanavir/ritonavir, darunavir/ritonavir)
    • medicines for bacterial infection (e.g., aminoglycosides, pentamidine or vancomycin)
    • NSAIDs to relieve pain in bones or muscles (like aspirin, ibuprofen, and naproxen)
    • medicines to treat fungal infection (such as amphotericin B)
    • anti-cancer drugs (interleukin-2)
    • medications for the treatment of viral infection (foscarnet, cidofovir or ganciclovir)
    • some other agents to treat hepatitis B virus (HBV) infection (like adefovir dipivoxil)
    • medicines for the immune system suppression (e.g., tacrolimus)
    • other anti-HIV medicines (didanosine, emtricitabine) as it may raise the levels of these drugs in the blood and reduce CD4 cell counts
