    Description

    Amifru® is marketed by Torrent Pharmaceuticals Ltd. and manufactured by Windlas Biotech Pvt. Ltd. This medication is a diuretic agent that works by helping you to pass more urine than you usually do. This reduces hypertension and stops the build-up of extra water in your body. Amifru® tablets contain two different active substances called Furosemide and Amiloride, both belonging to a group of medicines called diuretics. Furosemide is a sulfonamide-type loop diuretic used in the treatment of edema, hypertension and is also helpful in managing hypercalcemia. Amiloride, a potassium-sparing diuretic, is used to get rid of extra fluid and treat high blood pressure. It has the ability to store potassium in the body, which makes the compound usage of Amiloride and Furosemide beneficial so that not to disrupt the potassium levels of the body.

    Indications:

    - treating hypertension (high blood pressure);

    - edema associated with heart, liver, kidney or lung disease;

    - management of nephrogenic diabetes insipidus (should not be confused with diabetes mellitus or sugar diabetes), followed by abnormal production of urine (polyuria) and constant thirst (polydipsia).

    MOA (mechanism of action):

    The combination of the two active ingredients, Furosemide and Amiloride, is used in the treatment of hypertension by removing excess fluid from the blood vessels that reduces the pressure in the blood vessels, making it easier for a heart to pump blood around the body. The medicine causes the kidneys to eliminate extra salt and water from the body, thus reducing swelling and is especially useful for treating edema associated with heart failure, liver cirrhosis, renal impairment and nephrotic conditions when edema does not respond to thiazide diuretics. Furosemide, a loop diuretic, causes decreased renal vascular resistance and increased renal blood flow. Like thiazides, it can also lead to potassium shortage in the bloodstream (hypokalemia), it is hence used with Amiloride, a potassium-sparing diuretic, that reduces the potassium loss in the urine.

    Dosage

    The usual recommended dose is 1 - 2 tablets (40 - 80 mg/5 - 10 mg) per day taken on an empty stomach with a glass of water. Do not chew, crush or break a tablet.

    Missing a dose

    If you miss a dose of Amifru®, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed dose.

    Overdose

    If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately. Do not exceed the doctor’s recommended dose.

    Storage

    Store at a room temperature below 25°C (77°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the sight and reach of children.

    Contraindications

    Do not use the medicine if you have elevated serum potassium levels in your body.

    Do not take the medication if you have Addison’s disease (a hormone deficiency characterized by extreme weakness, loss of weight and low blood pressure), hypovolemia, hypokalemia, hyperkalemia, severe hyponatremia, dehydration, anuria, hepatic coma or precoma, diabetes mellitus, liver, gout or prostate problems.

    Do not combine Amifru® with the use of potassium supplements, stool softeners (laxatives), ACE inhibitors (ramipril, perindopril, enalapril), ARBs (losartan, irbesartan, candesartan), heart medications (digoxin, dofetilide), antipsychotics (lithium), muscle relaxants (diazepam, clonazepam, lorazepam), respiratory diseases drugs (theophylline), epilepsy drugs (phenytoin, carbamazepine), certain cancers medications (aminoglutethimide, cisplatin, methotrexate), anti-inflammatory drugs (aspirin, ibuprofen, indometacin, ketoprofen), immunosuppresives (ciclosporin, tacrolimus), ulcer medications (carbenoxolone), antidepressants (reboxetine), antifungals (amphotericin), corticosteroids (prednisolone), gout drugs (probenecid), liquorice, certain antibiotics (gentamicin, amikacin, neomycin, netilmicin, tobramycin, vancomycin, cephalosporins), asthma medications (bambuterol, salbutamol, terbutaline sulphate, salmeterol, formoterol) or other diuretics (triamterene, bendroflumethiazide).

    Do not take the medicine if you have a severe kidney disease or cannot pass urine.

    Do not use Amifru® if you are pregnant or breastfeeding.

    Do not take Amifru® if you are on potassium-rich or salt-restricted diet.

    Avoid consumption of alcohol as it may worsen the side effects.

    Not for use in patients under 18 years of age.
