    Description

    Isentress® is manufactured by MSD. The raw material is Raltegravir – belongs to a relatively new drug class called integrase strand transfer inhibitors (INSTIs).

    Indications:

    - treatment of human immunodeficiency virus type 1 (HIV-1) infection in adults and children weighing at least 25 kg, in association with other antiviral drugs.

    MOA (mechanism of action):

    Raltegravir targets integrase, an HIV enzyme that integrates the viral genetic material into human chromosomes, a critical step in the pathogenesis of HIV. The drug is metabolized away via glucuronidation. Raltegravir has shown activity against HIV-2 in vitro.

    Dosage

    Isentress® tablets must be swallowed whole.

    Adults:

    For treatment-naive patients, patients who are virologically suppressed on an initial regimen: 400 mg twice daily or 1200 mg (2 × 600mg) once daily.

    For treatment-experienced patients: 400 mg twice daily.

    For treatment-naive, treatment-experienced patients when co-administered with Rifampin: 800 mg (2 × 400 mg) twice daily.

    Children:

    40 kg and more: 400 mg twice daily or 1200 mg (2 × 600mg) once daily.

    25-40 kg: 400 mg twice daily.

    3-25 kg: only oral suspension or chewable form of Isentress® is recommended.

    Precautions:

    HIV can be transmitted through sexual contact with HIV positive partner, blood (e.g., sharing needles, handling blood instruments, receiving blood transfusions, blood products, or organ/tissue transplants that were infected with HIV), childbirth, toiletries (razors and toothbrushes).Therefore, both HIV-positive and HIV-negative individuals are advised to have any kind of sex using a latex or polyurethane condom, avoid sharing needles or other things like toothbrushes or razors. It is important to discuss with doctor the precautions needed to avoid infecting other people as well as take precautions not to get infected.

    Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

    Some people who take Isentress® may experience:

    - Liver problems, which may be life-threatening. Tell your healthcare provider immediately if you have any symptoms: dark (tea-colored) urine, yellowing of your skin or whites of your eyes, pale-colored stools, nausea, vomiting, pain or tenderness on your right side below your ribs, loss of appetite, or fatigue.

    - Skin rash, which may become severe or potentially life-threatening. Please seek medical advice immediately if you develop a rash.

    - Immune reactivation syndrome (severe immune deficiency during combination anti-retroviral therapy (CART) may lead to arousal of opportunistic infections and cause serious clinical conditions), autoimmune disorders. If you notice any symptoms of infection or other symptoms such as muscle weakness, weakness in hands and feet and moving up towards the trunk of the body, palpitations, tremor or hyperactivity, please seek medical advice immediately.

    - Osteonecrosis (the death of bone tissue). Patients should seek medical advice if they experience joint aches and pain, joint stiffness or difficulty in movement.

    - Muscle problems. Contact healthcare provider immediately if you experience unexplained muscle pain, tenderness, or weakness while taking this medicine.

    Missing a dose

    If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose of Isentress® to compensate the missed dose.

    Overdose

    If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store Isentress® tablets or oral powder at room temperature between 68-77 degrees F (20-25 degrees C). Keep out of the reach of children. Do not throw away any medications via wastewater or household waste. Ask your pharmacist how to throw away any medicines you no longer use.

    Contraindications

    Do not take Isentress® if you have a history of a muscle disorder called rhabdomyolysis, myopathy, increased levels of creatine kinase in your blood, phenylketonuria (PKU).

    Do not take Isentress® if you receive kidney dialysis treatment.

    Do not take Isentress® if you are pregnant or breastfeeding.

    Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

    Do not combine Isentress® with antacids (for indigestion and heartburn), rifampicin (for tuberculosis).

    Do not drive or operate machinery if you feel dizzy after taking Isentress®.

    Do not have any kind of sex without using a latex or polyurethane condom.

    Do not share needles or other things like toothbrushes or razors.

    Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.
