    Description

    Triomune® manufacturer is Cipla. Its active chemical compounds are Lamivudine, Stavudine that are synthetic guanosine nucleoside reverse transcriptase inhibitors (NRTIs), and Nevirapine – non-nucleoside reverse transcriptase inhibitor (NNRTI).

    Indications:

    - treatment of human immunodeficiency virus (HIV-1) infection in patients, who have been stabilized on the maintenance regimen of Nevirapine 200 mg twice a day, and have demonstrated adequate tolerability to Nevirapine.

    MOA (mechanism of action):

    Their mode of action of Lamivudine (3TC) and Stavudine (d4T) is to inhibit reverse transcriptase (RT) via DNA chain termination after incorporation of the nucleotide analogue. Nevirapine (NVP) directly inhibits RT by blocking the RNA-dependent and DNA-dependent DNA polymerase activities. Nevirapine has no effect on HIV-2 RT and eukaryotic DNA polymerases (such as human DNA polymerases α, β, γ or δ). Emtri is not effective against HIV-2.

    Dosage

    Triomune® 30 tablets for patients weighing less than 60 kg: 1 tablet twice daily.

    Triomune® 40 tablets for patients weighing at least 60 kg: 1 tablet twice daily.

    Do not use Triomune® in patients who have just initiated therapy with Nevirapine. Initial lead-in dosing of 200 mg Nevirapine once daily for 2 weeks is recommended.

    Triomune® should not be initiated in women with CD4^^ cell counts greater than 250 cells/mm^3^ or in men with CD4^^ cell counts greater than 400 cells/mm^3^ unless the benefit outweighs the risk.

    The 14-day lead-in period with Nevirapine 200 mg daily dosing has demonstrated reducing the frequency of rash.

    If rash continues after the 14-day lead-in period, do not increase the dose to 200 mg twice daily. The 200 mg once-daily dosing regimen should not be continued beyond 28 days, at which point an alternative regimen should be sought.

    Precautions:

    Patients must be monitored regularly during the first 18 weeks of therapy with *Nevirapine to detect potentially life-threatening hepatotoxicity or skin reactions.

    HIV can be transmitted through sexual contact with HIV positive partner, blood (e.g., sharing needles, handling blood instruments, receiving blood transfusions, blood products, or organ/tissue transplants that were infected with HIV), childbirth, toiletries (razors and toothbrushes). Therefore, both HIV-positive and HIV-negative individuals are advised to have any kind of sex using a latex or polyurethane condom, avoid sharing needles or other things like toothbrushes or razors. It is important to discuss with doctor the precautions needed to avoid infecting other people as well as take precautions not to get infected.

    Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

    Take special care if you have moderate or severe liver disease, you have ever had liver disease, including hepatitis B or C, you are seriously overweight (especially if you are a woman), you have severe kidney disease.

    Consult doctor before taking if you are taking phenytoin (for treating epilepsy), methadone (as a heroin substitute). Stavudine speeds up the removal of methadone from the body. If you are taking methadone, you will be checked for any withdrawal symptoms. Your methadone dose may need to be re-elevated.

    Some people who take Triomune® may experience:

    - Severe hepatotoxicity, particularly in the first 18 weeks, may develop in patients treated with Nevirapine.

    - Lactic acidosis and severe hepatomegaly with steatosis, including fatal cases, have been reported with the use of nucleoside analogues alone or in combination, including Lamivudine and Stavudine and other anti-retrovirals.

    - Severe skin reactions, including fatal cases, may occur in patients treated with Nevirapine. These are Stevens-Johnson syndrome, toxic epidermal necrolysis, and hypersensitivity reactions characterized by rash, constitutional findings and organ dysfunction.

    - Pancreatitis, including fatal cases, may occur in patients during therapy when Stavudine was part of a combination regimen that included didanosine in both treatment-naive and treatment-experienced patients, regardless of the degree of immunosuppression.

    Missing a dose

    If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose of Triomune® to compensate the missed dose.

    Overdose

    If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Contraindications

    Triomune® is contraindicated in patients who are allergic to Stavudine, Lamivudine, Nevirapine or any of the other ingredients of Triomune®.

    Do not combine Triomune® with other lamivudine-, Stavudine-, or emtricitabine-containing products.

    Triomune® is contraindicated if you have or have had very low red blood cell count (severe anemia) or very low white blood cell count (neutropenia), kidney disease or liver disease (such as hepatitis), peripheral neuropathy, pancreatitis, hepatitis B or C and, hemophilia, diabetes mellitus, erectile dysfunction.

    Triomune® is contraindicated in patients who are just starting therapy with Nevirapine. These patients require a lead-in dose of Nevirapine 200 mg once daily.

    Nevirapine is contraindicated for use as part of occupational and non-occupational post-exposure prophylaxis (PEP) regimens.

    Do not combine Triomune® with such medications astemizole or terfenadine (for allergy); amiodarone, bepridil, dronedarone, encainide, flecainide, propafenone, quinidine (used for irregular heartbeats); dihydroergotamine, ergotamine (for migraines); ergonovine, methylergonovine (for stopping excessive bleeding); clorazepate, diazepam, estazolam, flurazepam, triazolam or oral midazolam (help to fall asleep); clozapine, pimozide, (psychiatric drugs); quetiapine (for schizophrenia, bipolar disorder and major depressive disorder); lurasidone (for depression); ranolazine (for chronic chest pain); pethidine, piroxicam, propoxyphene (used to relieve pain); cisapride (for stomach conditions); rifabutin (for certain infections); voriconazole (for fungal infections); simvastatin, lovastatin (for lowering blood cholesterol); alfuzosin (for enlarged prostate); fusidic acid (for bacterial infections); PDE5 inhibitos (sildenafil, avanafil); colchicine (for gout), products containing St John’s wort, tavudine, ribavirin, valproic acid, fluconazole, probenecid, pentamidine, dapsone, pyrimethamine, cotrimoxazole, amphotericin, flucytosine, ganciclovir, valganciclovir, interferon, vincristine, vinblastine, and doxorubicin.
